Shoal Creek Baptist Association Van Guidelines
When picking up the Van, we will need the following:
A. When you come to pick up the keys, we will need a copy of the driver's license of
Whoever will be driving the van.
B. Name and contact information of the church's insurance company.
C. Information regarding driver's accident/ticket history. (Our insurance requires this
for coverage approval)
PLEASE DOWNLOAD THIS FILE AND FAX THE COMPLETED FORM TO
417-451-1527 AT LEAST 1 WEEK PRIOR TO YOUR TRIP
Before returning the van, please complete the following:
A. Wash the outside of the van and or the trailer.
B. Clean out the inside of the van or trailer thoroughly - this includes vacuuming the
van and cleaning up spills.
C. Please return the van to SCBA with a full tank of fuel.
D. Make sure to fill out the mileage log book for the van.
E. Report any mechanical or other problems to SCBA.
F. Turn in your $25.00 per day usage fee to SCBA.
